Applying to UK Medicine from the USA and Canada

Qualification requirements, cost comparison, and post-graduation career pathways

For American and Canadian students, UK medical schools offer an alternative pathway to becoming a doctor. Instead of the US route (4-year undergraduate + 4-year medical school), UK medicine is a direct-entry programme typically lasting 5-6 years from high school.

This guide covers what you need to apply from North America and whether it makes sense for your situation.

Why Consider UK Medicine?

Potential Advantages

  • Direct entry: Apply from high school rather than after a 4-year undergraduate degree
  • Time savings: 5-6 years vs 8+ years (undergrad + med school in the US)
  • Different admissions process: Less emphasis on GPA-building and extracurricular "checking boxes"
  • International experience: Live and train in a different healthcare system

Considerations

  • Costs are still substantial: £200,000-350,000+ over 5-6 years
  • Returning to practise in the US/Canada: Requires additional steps (USMLE, residency matching as IMG)
  • Distance from home: 5+ years far from family and support networks
  • Different career pathway: UK training structure differs from US/Canada

Qualification Requirements

For US Students

RequirementDetails
High School DiplomaRequired from accredited school
AP CoursesUsually need 3-4 APs including Chemistry and Biology
AP ScoresTypically 4-5 required (5 preferred)
GPA3.5+ generally competitive
SAT/ACTSome schools consider; check requirements

For Canadian Students

ProvinceTypical Requirements
OntarioGrade 12 U/M courses, 85%+ average, Chemistry and Biology required
British ColumbiaGrade 12 courses with strong performance in sciences
AlbertaGrade 12 (30-level) in required subjects
Quebec (CEGEP)DEC with science prerequisites; verify with each school

AP Equivalency: An AP score of 5 is generally considered equivalent to A-Level grade A/A*. A score of 4 is typically equivalent to B. Schools usually require at least Chemistry at AP level.

Cost Comparison: UK vs US/Canada

PathwayDurationApproximate Total Cost
US (state school, in-state)8 years$200,000 - $300,000
US (private undergrad + med school)8 years$400,000 - $500,000+
Canada (in-province)7-8 yearsCAD $150,000 - $250,000
UK (international fees)5-6 years£200,000 - $350,000 (~$250,000 - $450,000 USD)

The cost comparison is complex. UK may be cheaper than expensive US pathways but similar to or more than state schools with in-state tuition. Time saved (entering the workforce earlier) should also be factored in.

After Graduation: Practising in the US/Canada

Returning to the US

If you plan to practise in the US after UK training:

  • USMLE: You'll need to pass Steps 1, 2 CK, and 2 CS
  • Residency matching: Apply as an International Medical Graduate (IMG)
  • IMG match rates: Lower than US graduates, but UK graduates generally perform well
  • Timing: Many students take USMLE steps during UK medical school

Returning to Canada

  • MCCQE: Pass Medical Council of Canada Qualifying Examination
  • Residency: Apply through CaRMS as an IMG
  • Provincial requirements: Some provinces have specific IMG programmes

Staying in the UK

  • Graduate route visa: 2 years to work after graduation
  • Foundation training: Enter the UK Foundation Programme
  • Longer-term: Skilled Worker visa or settlement pathway

Key Insight

Plan your career pathway early: If you definitely want to practise in the US, study for USMLE during medical school and understand the IMG matching process. If you're open to staying in the UK, different considerations apply.

Schools Popular with North American Students

While most UK medical schools accept US/Canadian students, some have more established North American communities:

  • St George's: Historically popular with US students; some US-facing support
  • Edinburgh: Large international intake; many North Americans
  • Manchester: Large programme with diverse international cohort
  • Glasgow: Strong international community
